New patient care and risk assessment booklet

On Wednesday 04 October 2023 we will launch our new patient care and risk assessment booklet. This booklet is a crucial tool in our commitment to providing the highest quality care to our patients, and we want to ensure that all staff members are informed and prepared for this change.

The new booklet will have a few essential changes to the current booklets. There will be an enhanced what matters to you most section, a new purpose T pressure ulcer assessment, a simple oral hygiene assessment tool and a new discharge checklist.

QR codes are available on every page of the booklet that will take you to the information Standard Operating Procedure (SOP), when scanned. Here further information is available and examples of completed assessments and care plan from the knowledge experts.

The patient care risk assessment booklet is specific and individual to each patient and their needs. This is to be filled in by the healthcare professional that have seen the patient and the booklet will then be made available for everyone to read. This means when the patient moves to the next healthcare professional, this information will be instantly available, and care can be seamless.

On launch day, the new booklets will be delivered and once the new booklets have been issued, we will collect all of the old booklets as these will no longer be used.
